Recovery Program Approval – State Fiscal Years 2022 and 2023
The requests for approval form is now available for recovery programs seeking approval by the Minnesota Department of Education (MDE). Any Minnesota public school district and/or charter school offering recovery programs to students in recovery from substance use and/or co-occurring disorders may apply for approval. The process for becoming an approved recovery program includes submission of a biannual Request for Approval of Recovery Program Form to verify that the recovery program meets statutory requirements for approval as identified in Minnesota Statutes, section 124D.695, subdivision 1.
Recovery programs in Minnesota are not required to be approved by MDE to operate, however, only approved recovery programs are eligible to apply for a Recovery Program Grant of up to $125,000 a year. Grant funds may be used to pay for approved pupil transportation expenses and a portion of the costs for licensed support staff. Recovery Program Grant application materials will be available on August 23, 2021.
Submission and Questions
Please complete and sign the Recovery Program Request for Approval Form. Provide all requested information and supplemental documents, if any. Save all materials as Microsoft Word or PDF documents. These are the only file formats that will be accepted. The Recovery Program Request for Approval Form must be signed by the person with authority to sign on behalf of the applicant agency.
