Description: The training covers chronic condition management, interdisciplinary team approaches, motivational interviewing, student health care plans, and transitional planning.

Audience: The audience is school nurses, school health services providers, and other qualified professionals such as physicians, nurses, dentists, health educators, and other allied health personnel. Statewide Health Improvement Partnership (SHIP) staff are also welcome to attend if space is available. 6.50 CNE contact hours will be awarded to those that attend the entire day.
