Eligibility

Summer Food Service Program site participation may involve an area eligibility determination process. Household eligibility forms and letters, family size/income guidelines, instructions and question-and-answer documents are available to help sites determine eligibility.

Use the interactive and static eligibility maps to determine if sites are area eligible based on school or census data. If your site is not area eligible, contact our office for help with using other methods to qualify a site. Under Interactive Maps, select the Child Nutrition Program Eligibility Map. Click on the Summer Meal Programs option on the right side to determine if your site qualifies for participation. Note: Select "Hybrid" to verify the accuracy of the point and validate that the correct location is indicated on the map.

HUD and USDA-RD Subsidized Housing and SFSP Eligible Areas

The U.S. Department of Housing and Urban Development (HUD) and USDA Rural Development (USDA-RD) have joined with MDE to expand summer meals across the state in areas of large, multi-family housing. They seek sponsors for these sites to provide meals to the children who live in the area.

To identify eligible housing areas across the state, visit the HUD Map. The Housing Authority Documentation of Eligibility form may also be used to qualify sites.

 

Participant

  • Household Income Statement and Income Eligibility Guidelines 2021-2022  - 5/28/21
    Household letter, instructions and Household Income Statement form for distribution to households at enrolled sites (not area eligible) and camps that operate 7/01/21-6/30/22.
  • Household Income Statements Form-Distribution and Approval for Summer Food Service Program  - 2/25/21
    Requirement for camps and enrolled sites that are not area eligible to distribute the HIS packet to enrolled children. Procedure for sponsoring organizations to review and approve forms.
  • 9/3/21
    Requirements for using the option to approve Household Income Statement forms with an effective date in the month that the adult household member signed the form.

Site

  • Census Block Eligibility Form  - 5/31/18
    This form needs to be completed by sponsors who are hoping to become program eligible based on census data from adjacent block groups.
  • Public School List with Percentage of Free and Reduced-Price Eligible Students from School Year 2021-22  - 4/20/21
    To be used to identify schools that have 50% or more free and reduced-price meals qualifying the school as eligible based on school data. Eligibility applies to the 2022 Program Year for the Summer Food Service and Seamless Summer Program (May 2021-April 2022), Afterschool Snack (July 2021-June 2022), and the Child and Adult Care Food (Oct 2021-Sept 2022). Family Day Care Home eligibility is re-determined in February 2021 using this data.
